To be fair, Isshin was about as competent as Yuroichi and Urahara. It wouldn't make sense if he held any other rank as a Shinigami.

regardless of who called it, it's happening. that's good enough for me.

and we actually are getting an Isshin flashback more or less, though the lack of black around the borders of the panel isn't in here to show us that it's a flashback. and I wonder what happened to make him change his last name to Kurosaki...?
