He snuck up on them because of the other telepaths and Scott and Emma weren't paying attention.

There is a huge continuity error within the pages of the event. Some writers wrote the P5 as all TP's, TK's plus their active power amplified. Others wrote them as just their base powerset amplified.
Cyclops and Namor aren't telepaths as Phoenix under Bendis and Aarons pen. And Emma only has amped strength in Diamond Form. We even had Ross say that Emma is the only telepath amongst the P5.
Also that Hulk who is being blasted by Emma at the end is Red Hulk. And Emma was intrerrupted by Cyclops before finishing him off. We don't know how long she was fighting Red Hulk but him and Wolverine were hurt by her in UXM 18. And this is with her holding back.

Pretty sure that was intended to be Green Hulk being blasted at the end. Ucanny 18 portrays something totally different as is typical for tie-ins to events. In Uncanny 18 she is attacking Wolverine and Rulk while Cyclops walks up behind her while in the main book she seems to be in the air fighting green Hulk still when Cyclops flies at her from behind.
From what the issue said it wasn't quite as simple as they were holding back. The battle was from Emmas description apparently "going terribly" and the only way for them to win was to fully unleash and burn everything.

The main event had Hulk vs. Emma and we all know tie in, can be drawn from a different perspective.
Anyways, what wrong with Hulk tanking it anyways? Rulk could do it, than Hulk can do it twice as better.

That's what I thought but then I realized the Hulks hand that Emma is attacking mid-air is red.
She was attacking Wolverine from distance too so it's just a different angle.
Also, the battle was only going terribly because they weren't going to kill and it was testing their restraint, the more they were attacked the more both of them were forgetting why they didn't burn the world to ash in the first place. Which is something Rulk says in the Avengers tie-in. You're going to lose if you're not willing to kill. (Bendis wrote that too)
The Phoenix were fighting two fronts, keeping their restraint while the Avengers kept aggravating it. Like ants attacking a human and the human not trying to swat them (Again Bendis wrote that in the Illumanti issue). Like they said they are bigger then that.

It means Hulk was swatted some place off panel. Then Rulk and Wolverine came to help and Emma was in the process of dealing with them when Cyclops attacked her from behind. It's like completely different.
Also keep in mind Green Hulk is shown hitting Emma in human form in UXM 18 too. And that when Emma blasts Rulk they're in mid-air as Cyclops has to fly upward. So if anything that looks like Rulk was falling towards her and Emma blasted backwards while being hit in the back by Scott.
