He ducked. Clear as day and as he ducked the art even depicts him reacting and looking at SS mid-duck. The scan's right there and it doesn't get any clearer than that.

The scan is right there and it shows him ducking and reacting to Surfer. He cut off all sensory input and access to the gems so he did NOT have their power. This is obvious if you read the story and saw how he fought against the heroes before SS tried blitzing him.

He kept all of his new powers, but he could be surprised. That's it. He even sent Hulk through time, meaning the time gem was working just fine.
It's stated very clearly in the comics.

Yeah but he doesn't seem notice Surfer at all until Surfer tried to swipe at his gauntlet. Surfer was clearly going extremely fast too, why wouldn't he considering what was at stake?
Thanos didn't amp himself to that state, he made it so that he wasn't omniscient so that people could surprise him and so he could react accordingly.

It'd only make sense if Surfer went as fast as he could, if he fails the Universe is doomed.
Thanos cut off his omniscience didn't he? He might have god like stats but his ability to see things coming and reaction is what he primarily limited to give the heroes a chance.
