Exactly, when you consider Ironman you do it taking into account repulsor rays and everything he has with his equipment in most fights, time manipulation is among the tech Kang uses all the time despite the fact he has no powers. We have several instances of Kang using time-travel with just his armor -see Young Avengers for example.

Not for a while, especially since a ceiling hasn't been established for him yet. It takes months, sometimes years, to get a firm grasp on a character's power level. Patience is in order, at least for the moment.

Rulk is probably just a mini series.

Apparently Thor + Green Hulk > Rulk though. So hell, he might not be that psychotically uber. He still has all of 6 showings or so, so I'm against placing him at all, but factor in Thor's versatility and even if he gives up a little in brute force, he's probably not any higher than Thor's High Herald...which of course will likely infuriate Hulk fans, but I'd say it's an honor to share a tier with people like Superman and and Surfer

With help from a High Herald. Immediately after another fight with Thor, and crashing into Earth from the moon. So no.

Also: Red Hulk: "Unbelivable. I'm big enough to admit that Thor had me on the ropes--and in your stupidity, you may have just spared my life"

...so it's possible Thor alone would've done it on his 2nd try.
