I don't mind the sporadic cameo of him if it serves a purpose outside of "_________ fights Doom this issue!" or if he's important in a specific arc/storyline in an ongoing. But really, Doom has his hands in so many different niches on Marvel Earth, so it's bound to happen.

Huh?
If by common fan you mean people who don't really follow comics but watch tv and movies with those characters, I can see that, I suppose.
But to comic book fans, Doom is easily the biggest and most infamous villain in Marvel. He certainly is in-universe as far as Marvel Earth is concerned as well.
