Don't know. Of all the rumors circulating the movie, what's revealed in this force vision/flashback is easily the most mysterious. However, I'd guess that the Knights of Ren are responsible.

Well let's hope they learned after the Darth Maul disaster. Granted they eventually retconned it but it was long after rots and he was out of the films which is what the majority of the fans IMO wanted. Instead we got Dooku for two films. Dammit, Lucas.
