as far as in-universe explanations, it could have been prevented if they realized that the war served the sith purpose and that clone troopers are inherently highly dangerous
that takes wisdom. zao for one actually objected to jedi participation in the war, to Tholme.

The Jedi have faced many threats over the millennia, from Sith, to Mandalorians, to more Sith. But no one Jedi High Council managed to let the Republic get more ****ed up than Yoda, Mace Windu, Saesee Tinn, and the other Jedi during the prequel trilogy. Failing to notice the return of the Sith? Check. Allowing the Separatist movement to start? Check. Refusing to train the most powerful potential Jedi in millennia? Check. Missing the creation of a giant clone army? Check. Constantly shitting on the overemotional teenaged version of the most powerful potential Jedi in millennia thus ensuring he has every reason to hate Jedi? Big check. Allowing the galaxy to erupt in civil war? Check. And most importantly, never noticing that the most powerful Sith in the galaxy has been sitting like three blocks away for almost a decade? Check and mate. The Jedi High Council of the prequels managed to get every single thing they did wrong, thus ensuring the rise of the Empire and every death that occurred in all six Star Wars movies. No wonder Luke didn’t listen to you on Dagobah, Yoda. Sure, you can lift an X-Wing out of a swamp, but at least Luke can recognize a bad guy when he sees him.

The High Council has plenty of more dumb feats to their name.
In their long-standing tradition of pissing off the most powerful Jedi in a millenia, they ignored Anakin and expelled Asohka Tano when it was clear she was framed.
Couldn't understand what Barriss Offee clearly saw: They were pawns for the Sith and the entire Order had become corrupt.
They refused Anakin's promotion to Master just because Palpatine wanted it, not because he totally earned throughout his Clone Wars exploits.
Without informing the Senate, the Clone Troopers, the Coruscant PD, anyone, they only sent in three Jedi to arrest the Sith Lord who's been running the entire government for the past 15 years. So when those three failed, the Jedi were easily wiped out without anyone knowing Palpatine's secret.
They employed thousands of troops that Sifo-Dyas secretly ordered, without even checking with the Kaminoans or like, anyone if they had any secret executive orders implanted.
Finally, too many meetings. Way too many meetings. They probably couldn't sense danger because they were too busy updating their Outlook calendars.

Blows it, Yoda does. If you really want to know whose fault it is that the Old Republic and its democratic ways get tossed aside in favor of the Empire that Luke and Leia have to rebel against, look no further than Yoda.
Supposedly the wisest of all Jedi, he couldn't sense that the Republic's most power-hungry senator, Palpatine, was actually a Dark Lord. Palpatine gains power by putting his apprentice, Count Dooko, in charge of a group of rebels. Then he asks the senate for an army of Stormtroopers to defeat the rebels. Yoda not only goes along with it, he leads the Stormtroopers into battle.
