Kisame is indeed inside Samehada, while the dead kisame was a Zetsu clone. He sneaks into Kumogakure & decides to attack the 8-tails as the latter is alone.
Kabuto has control of Orochimaru, or so it seems.
Cover has a different looking Kabuto with weird puppet-like thing behind him

As for that Kunai thing, That's how Sasuke would have won against Naruto 313

Nah. Pretty potent one, but Kakashi said that Orochimaru likely treated Sasuke for resistance to most poisons already.

Although Sakura didn't accomplish a lot recently (well, saved Karin. But hasn't won a fight since she beat up the giant summoned centipede), between this and the gas bomb, her chemical skills seem to be getting pretty impressive, if she can drop even Naruto with them.

In the Mangastream translation, Sakura claims that Naruto's resolve is nothing compared to hers, yet her later words imply otherwise.

Kabuto was strong to begin with, but he always lacked a special edge to kick him properly into the S-class. An edge which now he has.

Thing I find the strangest--how exactly doesn't Samehada know that Kisame is physically inside of it? I mean really how card is it to tell something like that.

It might know. It might also not care/be able to tell anyone. Samehada does what Samehada wants.

Not exactly, I think that was him for everything but the end, when it was White Zetsu who got decapped.

They made the switch in the water, probably towards the finish. All the big jutsu were Kisame.
