First, Hulk smiling was the panel after Cap used the pressure point, he does have a healing factor you know.
As for the Batman example, unless I'm mistaken, that was pre-crisis

If you'll notice in the first scan, the shield wasn't blocking all of it and in the second scan he stops using it to block and is thus taking the full heat.

Really now? I call it a very, very low showing. I have Batman killing a leopard and beating on a tiger, actually. The real moral here? Comics hate PETA.
Anyway, losses don't matter if Cap's never fought the people Batman lost to. Without his shield, he's never fought ANYONE of repute, much less someone ont he level of even the guys Batman beats the hell out of every other issue. I don't know if you've ever done any kind of formal debate... but the burden of proof ALWAYS falls to the positive. You claim that Cap is as good as Batman without a shield, it is not my job to prove that he isn't, if you have nothing that says that he is.
PIS contest!!! Woohoo!!! Hulk, you say? First off, Wonderwoman is more straight up durable than a calm Hulk is. She's been punched from the sun tot he Earth, without a whole lot of damage done. Grundy trades blows with Superman. And now, Batman figths Luthor, wearing armor, and hopped up on venom and liquid kryptonite. Before figthing Batman, he had brawled with Supes.
Back to Cap in a furnace...
Batman is trapped in a building. The building explodes. Does Batman go with it? Hell no. He also takes getting set on fire like an absolute badass.

Meh, I'd go as far as to say that Batman's gotten stronger since the first Crisis. Old Marvel comics are often just as inconsistant as Pre-Crisis DC comics from the same time period...

Includes the one that Mr. Master talked about earlier... A girl fell 10,000 feet with a malfunctioning parachute. It wasn't exactly terminal velocity, but her speed was estimated at about 50mph. One guy fell from 3,500 feet when neither of his parachutes opened, and landed on (and went through) the roof of a hut. There's a whole bunch of similar stories on there. None of them come off quite as cleanly as Cap, but most are from higher heights than those in most of the scans as well.

Furthermore, some of those scans are not even Classic Cap, and the only scan you posted that Cap does not have his shield, is when he's beating on an inanimate object(steel door). All my scans are hand to hand displays of Batman's prowess and here's some more(Hand to Hand).
Silent Master posted a site that shows many cap scans (more than 30). You will see cap putting a hurting on a lot of powerhouses. caps fights with his shield mostly when he's fighting super powered enemies like hulk, terrax, etc. He fights (super powered) enemies more than 90% of the time. If you go to the scan site Silent Master listed then you will see Cap beating several super powered enemies without using his shield. I assure you that when cap mostly fights non-meta humans straight up (like iron fist, crossbones, red skull, etc.) he hardly ever uses (or needs) his shield. Batman on the other hand don't fight enemies like cap do in the majority (more than 50% of the time). Batman fights more thugs and other humans more than super powered people. He is mostly a crime fighter. Cap has taken direct blows from the cream of the crop (like hulk) and still was in battle.
This all said not to bring down Batman but to show why cap uses his shield alot and that he still has the ability to kick major butt without it.
Thus Silent Master made no mistake.
Lastly, what does classic cap has to do with anything here? Cap is always in continuity. Using any version of cap is acceptible since he didn't lose any abilities over the years.

that's not batman owning him. I have that comic (its a crossover) and batman had to reason with hulk in order for hulk to stop. Otherwise, batman was a dead man. Also, that was pre-crisis batman. Batman is not in continuity like cap is.
If you looked at the both of these characters and their stats, you'd see that this thread should never have happened, Batman is more on Daredevils level while Deathstroke Terminator is more on Captain America's level (and Deathstroke pummeled Batman about 7 yrs ago, telling Batman that he was trained to fight while he was trained to kill)
Captain America only lost to Bats because of a vote back in the DC vs Marvel thing, the truth is Cap shouldn't even break a sweat kicking this guys a**. In JLA vs The Avengers while they were on the roof Bats says, "It's conceivable that you could beat me, but it would take a long time" bogus!!! Cap would have grabbed him by that dumb ass cape and threw him of the roof. DC just wanted to protect one of their flagships, but we all know what would have happened in less than five minutes time. The Batman goes down. HARD!
Actually on the DC boards Kurt said that Batman was admitting that Cap was the more skilled.
