I've looked at the art. It looks more like Hulk grabbing mjolnir away and hitting Thor with it. No conclusive panel showing Thor's hand is wrapped inside Hulk's. And why would Thor let Hulk swing his arm around like he's a little boy? Doesn't make sense.

i think Rage post an interview with the writer and if i'm not mistaken..the writer said Hulk grab Thor's hand..Thor is still the one holding Mjolnir..and the writer like Hulk more..he said an enrage Hulk is stronger than Thor..Hulk is the strongest there is..

On the fourth page Thor's right arm is in Hulk's fist with Mjolnir. As I earlier mentioned, I do think it looked like he snatched it out of Thor's left hand, and his wrist band even looks like part of the handle, but it ended up not being the case. And I also think it's dumb that he just sat there. He could of easily flipped Hulk around if he was going to be hanging on like that, but he didn't. Point being - loophole found.
That's a badly drawn panel, if you think Thor's hand is inside Hulk's fist. His forearm looks so small compared to Hulk's, it looks more likely that his arm is pointing off to the right, with hulk's fist looking bigger in the foreground.

I didn't even notice it was in his hand. I was looking at the bottom of the second page pretty hard picturing how they were swinging and saw the hammer was in his left hand so I just assumed it was in his left hand the whole time. And when he grabs Thor's fist it looks like he snatched the hammer out of his left hand.

That doesn't seem more likely at all. I don't disagree that the art is weird but in that top panel Hulk is wrapping his hand around Thor's, which makes the next panel's intention pretty clear.
It looks like Hulk's hand is closing around the hammer in Thor's hand to take it away, to me. There's no conclusive shot of what you speak of.
Did they draw it that way on purpose?

I think he means it would be pretty anti-climactic. Thor was helpless when Void destroyed Asgard, hell he was terrified when Void had him in his grasp.
