Slade had no real intention of fighting Bats when they first encountered each other, and only fought him because Batman forced him to. He's also physically and mentally superior to Batman, has knowledge of who he really is and has considerable resources and connections of his own. Mentally superior in terms of reaction time, the ability to analyze information at a far greater pace, using 90% of his brain, etc.
Batman is smarter, yeah, and is more skilled, but the physical and mental edge Slade has on him coupled with his own equipment and skills makes him superior to Batman in many ways.

Slade has beaten people superior to Batman, including Batman himself.
I think everything Slade brings to the table makes him an opponent Batman would rather not add to his regular rogue's gallery if he could help it.
In a h2h encounter, which took place 2 decades ago...
Since then, Bats has accumulated a slew of ridiculous feats in terms of skill and physical stats.
But putting all that aside and granting Slade the physical edge, Batman has the advantage of using prep, intellect, and resources to get the job done, whereas DS usually relies too heavily on his fists, sword, and gun. There's a reason why Superman called him the Earth's most dangerous man.
If DS was a part of his rogue's gallery, I'm sure he'd handle him just fine.

Batman doesn't face anyone like Slade in his rogues' gallery. He can't even beat him h2h.

My verdict: Batman could handle Slade but he'd lose pretty much every straight-up blind-alley encounter and he'd stalemate more often than not and I can't see Slade going in and out of Arkham or Blackgate like the rest of the Batman rogues.

Regardless, like I said, even if I grant Slade the physical edge (though they have comparable feats), it's going to take a lot more than that for somebody to be considered Batman's "worst nightmare." The guy takes down much more formidable threats on a consistent basis (as you've acknowledged).
Not to mention, even if Slade was part of Batman's rogue gallery, he wouldn't even be Batman's most dangerous foe.
